Epithelial Keratitis

Patient Case Overview

Patient arrived in office complaining of a sensation of severe burning & pain in her eyes. Under slit lamp she had significant positively staining epithelial keratitis that was worse in the right eye.

Eclipse placed immediately after presentation
Day 3: amnion fully degraded, CL still in place
Right eye immediately after removal of soft contact lens
Right eye stained w/ fluorescein on Day 3 — initial defects resolved, tear film normal
Clinical Notes

Large central epithelial defect with loose epithelium at the edges. VA 20/200 with severe discomfort. Central corneal injury with risk of visually significant scarring. Debridement was not done, Eclipse placed with BCL on top, Maxitrol drops BID with Refresh Optive Sensitive q3h w/a, 5 days later BCL removed, VA 20/40 (due to cataract) with zero discomfort. Full healing with zero scarring.

Conclusion

This case demonstrates the effectiveness of amniotic membrane therapy in treating epithelial keratitis. Following placement of Eclipse, rapid epithelial recovery was observed, with full degradation of the amnion and resolution of initial defects within days. The patient achieved restored epithelial integrity and normal tear film distribution, confirming the value of early regenerative intervention in acute epithelial conditions.
